Terms & Conditions

1. Introduction & Purpose

Welcome, and thank you for choosing Houda Alkalla & Changing Minds, Changing Lives™ LLC.
This document serves as the complete Client Agreement, Terms & Conditions, and Privacy Notice governing our work together.

By signing below, you confirm that you have read, understood, and agreed to all terms outlined in this document. This agreement exists to ensure clarity, safety, professionalism, and mutual respect throughout our work together.

 

2. Scope of Services (Not Medical or Mental Health Treatment)

I provide coaching, hypnotherapy, subconscious reprogramming, and transformational services intended to support personal growth, self-awareness, emotional regulation, and well-being.

You understand and agree that:

  • These services are not medical, psychological, or psychiatric treatment.

  • I do not diagnose, treat, cure, or prescribe for any medical or mental health condition.

  • These services do not replace care from licensed medical or mental health professionals.

You are responsible for maintaining appropriate medical and psychological care and consulting licensed providers as needed.

Certain conditions (including epilepsy, psychosis, or untreated severe mental illness) may make participation inappropriate. By signing this agreement, you affirm that you do not have such conditions, or that you have fully disclosed them prior to beginning services.

Additional Educational Content Clause

Educational materials, including discussions related to nutrition, supplements, lifestyle practices, nervous system support, or wellness strategies, are provided for informational purposes only. Such information is not medical advice, diagnosis, or treatment. Clients agree to consult with a licensed healthcare provider before starting or modifying any supplement, medication, or health-related practice. Participation in coaching or hypnotherapy does not replace medical or psychiatric care.

Clients understand that coaching and hypnotherapy services are complementary and supportive in nature. They are not psychotherapy, counseling, or mental health treatment. Clients experiencing severe depression, suicidal ideation, psychosis, trauma-related instability, or other acute psychiatric symptoms agree to seek appropriate licensed mental health care. If such concerns arise during our work, referral to a licensed professional may be recommended.

 

3. Client Responsibilities

To support the effectiveness and safety of our work together, you agree to:

  • Be honest, open, and fully present during sessions

  • Complete any assigned practices or exercises when applicable

  • Inform me of any significant changes in medical or psychological status

  • Attend sessions on time and from a quiet, private, distraction-free environment

  • Use any personalized recordings for personal use only, and never while driving or operating machinery

 

4. Practitioner Responsibilities

I commit to:

  • Maintaining professionalism, integrity, and confidentiality

  • Providing a safe, respectful, and non-judgmental environment

  • Being prepared and on time for scheduled sessions

  • Responding to messages within 24 business hours, excluding weekends and holidays

 

5. Program Structure & Scheduling

Coaching and hypnotherapy services are typically offered in structured programs (commonly a 3-month program of six sessions, each lasting approximately 90–120 minutes, with support provided throughout the month), unless otherwise agreed in writing.

  • Packages must be used within six (6) months of purchase unless otherwise stated in writing

  • Late arrival may result in a shortened session time

  • Arrivals more than 15 minutes late may be considered a no-show

 

6. Payment Terms & Refund Policy

  • Payment is required in advance, no later than seventy-two (72) hours before a scheduled session or program start date.

  • Missed or late payments may result in suspension of services until payment is received.

Refund Policy:

  • No refunds are provided for completed sessions

  • If a prepaid package is discontinued, a pro-rated refund will be issued for unused sessions, calculated at the single-session rate, which is used solely for refund calculation purposes.

  • Results are not guaranteed; outcomes depend on the client’s participation, commitment, and follow-through

 

7. Cancellations, Rescheduling & No-Shows

  • A minimum of 24 hours’ notice is required to cancel or reschedule

  • Cancellations made with less than 24 hours’ notice are non-refundable

  • Missed sessions without notice (no-shows) are forfeited

 

8. Confidentiality & Privacy Notice

All sessions are confidential, with exceptions only when required by law, including:

  • Risk of harm to yourself or others

  • Abuse or neglect of a vulnerable person

  • Legal requirements such as a court order

I may discuss cases anonymously for professional supervision, consultation, or training purposes.

Privacy Practices:

  • Information collected may include name, age, contact details, emergency contact information, and brief session notes

  • Records are stored securely using password-protected or locked systems

  • Records are retained for three (3) years and then securely destroyed

  • You may request access, correction, or deletion of your records within legal limits

I am not a HIPAA-covered entity; however, reasonable safeguards are used to protect your personal information.

 

9. Telehealth & Electronic Communication

Sessions may be conducted via Zoom, phone, email, or text. You acknowledge and accept the inherent risks of electronic communication, including privacy interruptions or technical issues.

If technical difficulties occur, reasonable efforts will be made to reconnect or reschedule.

 

10. Recordings & Intellectual Property

All recordings, scripts, handouts, and materials provided remain the intellectual property of Houda Alkalla & Changing Minds, Changing Lives™ LLC.

  • Materials are for personal use only

  • They may not be copied, shared, distributed, or resold

  • Personalized recordings must never be used while driving or operating machinery

 

11. Standards of Conduct & Termination

Sessions will not be conducted if you are under the influence of drugs or alcohol.

Abusive, threatening, or disruptive behavior may result in immediate termination of services without refund.

Either party may terminate this agreement with 72 hours’ written notice.

 

12. Force Majeure

Neither party shall be held liable for delays or cancellations due to circumstances beyond reasonable control, including illness, accidents, natural disasters, power outages, or internet failures. Sessions will be rescheduled when possible.

 

13. Limitation of Liability & Indemnification

My total liability for any claim related to services is limited to the amount paid for services during the three (3) months preceding the issue.

I am not liable for indirect, incidental, or consequential damages.

You agree to indemnify and hold harmless Houda Alkalla & Changing Minds, Changing Lives™ LLC from any claims, liabilities, damages, or expenses arising from your participation, except where prohibited by law.

In no event shall Houda Alkalla or Changing Minds, Changing Lives™ LLC be liable for any medical decisions, supplement use, lifestyle modifications, or health-related actions taken by the client.

 

14. Governing Law & Dispute Resolution

This agreement is governed by the laws of the Commonwealth of Virginia, USA.

Both parties agree to attempt resolution through good-faith discussion first.
If necessary, non-binding mediation may be pursued in Virginia.
Unresolved disputes shall be handled in Virginia courts.
